traffic collision on the intersection with lights

Nearly one week ago when i have returned to home I stopped my car at a red light, next after the change of lights I was going to turn to the left, then collide my car I lost consciousness and was in hospital one night. There weren't the wittnes in self-defence. The police came and  to drew up a memorandum. Who sombody should I report to?

What's happend exactly?  I understood you aren't a culprit of event. If the police came on the place accident so they made up a police memorandum. If they don't posses your name, not  data available, you must go to the nearest a police station and you will ask them for note with informations from accident place.
