broken nose and knocked out tooth

I was injured in accident at work two weeks ago. I have problem because I think that this accident wasn't my fault but my boss say that I was unguarded and I'm respond for this accident and I can't claim compensation. I had broken my nose and knocked out a tooth. The employer has a full insurance. Can I report to his insurance? what kind of documents I need? The police didn't come but a witness made a declaration, besides I've all medical documentation.
